About the Department
The Health Independence Program (HIP) at SVHM consists of Complex Care Services (CCS), Sub-Acute Ambulatory Care Services (SACS) and the Inner North Melbourne Post Acute Care Service (based at North Richmond Community Health (NRCH)). HIP offers a comprehensive range of specialist clinics, services and rehabilitation programs, providing rapid response health management, short – medium term care coordination and health education. HIP enables clients to be the decision makers of their care, and support clients through self-management to achieve their healthcare goals.
About the Role
This position is for a Clinical Psychologist to work part time up to 16 hours per week within the St Vincent’s Hospital Health Independence Program (HIP) until August of 2018. As a clinical psychologist within Complex Care Services, you will provide primary consultations with clients to perform clinical diagnostic assessments and establish a management plan as part of the interdisciplinary team. Intervention and short term management aims to optimize psychological function, encourage self-management and promote empowerment for clients to improve their health outcomes, well being and quality of life. Psychological intervention is prioritised for those clients experiencing psychosocial issues that impact on safety, functional independence and impose barriers to engagement, self-management and positive health outcomes, and includes identification and referral to appropriate ongoing psychological support. Secondary consultation to colleagues and the provision of formal education across HIP is an essential component of the clinical expertise that the psychologist provides.
• Direct clinical provision of high quality and evidence-based psychological assessment, intervention, education and management
• Undertake specialist assessment of the person’s psychological enablers and barriers
• In consultation with the interdisciplinary team, formulate management plans that take into account a client’s strengths and limitations
• Recommend and/or provide appropriate clinical intervention and/or refer on for longer term, community based psychological support
• Lead and/or participate in group-based rehabilitation and intervention programs
Qualifications, Skills and Experience
• Masters or Doctorate Degree in Clinical Psychology recognized by the Australian Psychological Society
• Current Australian Health Practitioner Regulation Agency (AHPRA) registration as a Clinical Psychologist
• Extensive experience as a Clinical Psychologist, working with clients experiencing psychological issues, multiple health issues and complex care needs that impact safety, function, independence, self-management skills and health outcomes
• Experience in the community, addressing the needs of clients living with chronic diseases and/or complex health needs or psychosocial conditions
• Specialized understanding and scientific approach to human behaviour, cognitive performance, mental illness and Psychosocial problems
