About the Department
Palliative Care is the active total care of people whose disease is not responsive to curative treatment. Care is delivered by coordinated medical, nursing and allied health services that are provided, where possible, in the environment of the person's choice.  The inpatient units are across two sites, Caritas Christi Kew with its 26 beds and St Vincent's Fitzroy and its 8 beds. This position is located in Kew on the Caritas Christi campus however at times, may be required to attend the Fitzroy campus.
 
About the Role
We are seeking a Registered Nurse with proven experience and clinical expertise in palliative care for the admissions and coordinator role. You will be located at Caritas Christi in Kew, working for two days per week and job sharing the alternate three days.
 
You will be responsible for managing palliative care referrals and admissions for the inpatient beds and palliative day hospice, maintaining PCOC data and participating in improvement activities.
 
It is essential that you have highly developed interpersonal, time management and communication skills with the ability to work effectively both independently and with the multidisciplinary team.
 
Duties
  • Triage referrals for Palliative Care suitability, assessing medical, pharmaceutical, nursing, client and families’ needs to prevent inappropriate hospital admissions
  • Ensure that the systems to manage referral and admission processes are functional and enable excellent relationships with all referrers
  • Maintain PCOC data across St Vincent’s
  • Ensure the operational efficiency of the Day Hospice and coordinate referrals and admissions
  • Performance manage staff in the Day Hospice
Qualifications, Skills and Experience
  • Registered Nurse Division 1
  • Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Clinical expertise in an inpatient and Community Palliative Care Setting
  • Demonstrated ability to make sound clinical decisions
  • Excellent communication skills and ability to share knowledge with other clinicians across multiple disciplines/settings
  • Demonstrated ability to work autonomously
